COVID-19 VACCINE INFORMATION

The City of Haines City works with various organizations to make COVID-19 testing and vaccinations more readily available and accessible to our community. If you have any questions regarding the following information, please contact each organization directly.

As of Monday, April 5, 2021, all Floridians are eligible to receive the COVID19 vaccine. Pfizer vaccine is authorized for persons age 16 and up. Moderna and Janssen (Johnson & Johnson) are authorized for persons age 18 and up. All individuals under the age of 18 receiving a vaccine must be accompanied by a guardian and complete the COVID-19 vaccine screening and consent form. To download a copy of the form, click here.

WHERE CAN YOU GET A VACCINE?

  • Please use the vaccine locator tool (link below) to view locations and make appointments with retail pharmacies (e.g., Publix, Walgreens, CVS, Wal-mart). | Vaccine Locator
  • Appointments may also be available through the Polk County Department of Health. Please call the Polk DOH Vaccine Hotline at (863) 298-7500.
  • FEMA will be administering vaccinations through pop-up locations. These COVID-19 POD locations are first-come, first-served only, and appointments will not be made. 
    • Currently Scheduled:  FEMA is scheduled to return to the Oakland Neighborhood Center April 8-13 to administer second doses only.
